This is only an application form. Sacred Heart College reserves the right to accept or reject this application. The information requested in this form is held in the strictest confidence.

Copies of the following must accompany this application:

Identity Documents of the parent(s)/guardian(s)/person(s) responsible for fees
Identity Document or Birth Certificate of the applicant (student)
Latest annual and termly school reports of the student from his/her current school
Last 3 months' bank statements
Last 3 months' payslips
Passport-size photograph of the applicant
Relevant Visa, date of entry stamp & passport front page (international students only)
Baptismal certificate (if Catholic)

▪ A non-refundable application fee of R500.00 is required when the application is made.

▪ If accepted, a non-refundable placement fee confirms acceptance of the offer.

▪ Once submitted, the application form (and all relevant documentation) becomes the property of Sacred Heart College and will not be returned.

▪ Please use block capitals when filling in the form.
